Not exactly a hardware mod....but darned fun!
I just finished this project.....
Built with MDF wood with 1x1 struts for support in the corners. The control panel was built with parts from Happ Controls, and an X-Arcade control board with a Dreamcast adapter. I was going to hack a couple of controllers, but didn't feel comfortable enough in my soldering skills (or lack there of). The controls work great. The only downside to this method is the loss of memory card functionality. To resolve this, I took apart a DC controller that no longer worked. The controller still read memory cards. I took out the circuit board from the controller, built a box around it with a hole exposing the memory card slots. This is plugged into port 3, giving me access to memory cards on that port.
Took about 1 week and a half, and cost was a little over 400 bucks (And that includes the 19" TV and Dreamcast that I already had). I know it doesn't add anything cool to what the DC already does, and it really isn't a mod to the DC itself....but it sure looks good in the game room and is a heck of a lot of fun to play Street Fighter 3 on real arcade controls

I like it, it looks awesome.
i thought about building the dreamcast in a arcade case for a long time, but never got the spare money to do it.
PS: i miss 2 lightguns on this cabinet

It's great! Almost perfect - except that you should have used a VGA box and a PC monitor instead of a TV and RGB lead

Yeah, ideally I would have liked to use a VGA monitor, but I used the two 21"s I had for the other MAME cabs.
I got really lucky a few weeks ago when the IT department at my company was selling off all the CRT monitors after we switched to LCD. I ended up buying 2 21" monitors for a buck a piece. So I took em home, decased them and dropped them into a couple of MAME cabinets I had built awhile ago using smaller monitors.
The only real mark against it is the lack of analog controls.
I got really lucky a few weeks ago when the IT department at my company was selling off all the CRT monitors after we switched to LCD. I ended up buying 2 21" monitors for a buck a piece. So I took em home, decased them and dropped them into a couple of MAME cabinets I had built awhile ago using smaller monitors.

Would have been nice to be able to play stuff like Shenmue, etc on it.
But a Happ analog stick is like $150!
That's almost half of the entire cost of the project

Very nice.
I often wake up in the night thinking there is a cab just like that standing in the corner of the room ! .
Unfortunately it's usually my beautiful GF standing there dressed in basque and suspenders asking permission to 'relieve' me My life is full of disappointments .
