Please tell me what other information is needed. Which software to use to get the checksummes files (Hash, MD5, etc...)?

Hello Lin and thanks for your first submission. :)
The info you are giving is mostly sufficient. The only extra thing that is absolutely necessary is the ringcode info from the disc.
This can be found on the inner rim at the bottom of the disc and looks something like T-20106M-0655 3 or <T-1213M-0313>HK121D or T-3002M-0501 3M1 C 0Y for japanese discs. In any case, just make sure you include all the letters and numbers readable on the bottom of the disc and I can distinguish the patterns...

As for hashing software, there's no "must". I personally find ClrMAME Pro convenient: http://mamedev.emulab.it/clrmamepro/
In the "about" box you can drag and drop any single file and it will spit out its' hashes, or you can setup a "hashing" dir and it will create a .dat with checksums for anything found in there. Don't forget to include filesize, crc32, md5 and sha1 values.
Simple lines like below are more usable for me, but if you find any other style better, I can manage. :)

OK, so the ringcode in the pic is <610-7967-0567>HKB09D so that's sorted. :)
If you have a hard time reading them, post pics like this one and I'll "translate".

But as for the hashes, while the needed info is there, I'd like them in (any kind of...) text format not as an image. Copying a large hex string character by character from an image so I can compare with another dump or enter in the dat is error prone and very time consuming for me. Not to mention the fact that Imageshack usually drops images after a while, so by the time another dump of this surfaces the original info you posted in the image might be lost forever.
