Codebreaker code format

I've worked with the PSO codebreaker stuff in the past, so I have some basic knowledge on the format, but is there a document or guide anywhere detailing the other types of functions the code headers can perform for the version of CB the DC uses?

Also, how are the "check" values calculated for entries that span multiple lines? In the PSO app, they're constant, so I'm guessing it's a 16-bit checksum only on the 32-bit starting address? What polynomial is used for it?
