To answer you questions, in order:
1. Yes, you do need the PSO Patcher v1.1 to play any Japanese version of PSO for Dreamcast. All of them had the Hunter's License check, so you need to disable it. You could use a codebreaker for this purpose, but its easier to just use the PSO Patcher disc. The only versions that didn't have the Hunter's License check are USv1, EUv1, and EUv2. I recommend using the PSO Patcher disc with any v2 versions of the game anyway, since it includes the Ultimate-mode mapfix patch. 2. Yes, you can play it on a non-Japanese console. The PSO Patcher disc will allow you to boot imports, so there's no difficulty with that. 3. Yes, you can switch the in-game language to English. Its in the Options menu when you start the game up.
